There are several options or decision points along the patient pathway. In some clinical situations, there is very little time to wait for ear drops to work. In other circumstances, using a treatment for several days is a clear option. In some settings, ear syringing or micro‐suction are readily available, and the use of drops is seen as a prelude to one of these interventions. We also rated the quality of evidence for our other primary outcome, adverse effects: discomfort, irritation or pain, as low for all three main comparisons, meaning that we have limited confidence in the effect estimate and the true effect is likely to be substantially different from the estimate of effect. Only one study compared using drops with an active ingredient to not using drops at all. The drops may help increase the proportion of ears cleared of wax from 1 in 20 (if you do nothing) to about 1 in 5 (if you use drops). Cerumol ear drops are used to soften and remove hardened ear wax build-up. Arachis oil (peanut oil) and chlorobutanol are ingredients in Cerumol ear drops. When you place the drops in your ear, the ingredients soak into the hardened wax and soften it, allowing it to drain. Your doctor or practice nurse can also remove a hard plug of wax by cleaning the ear with warm water. If there is a considerable amount of wax or the wax is hard, it may need to be softened first, which Cerumol ear drops can help with. Cerumol ear drops contain arachis oil (peanut oil) and chlorobutanol. When you put the drops in your ear these ingredients soak into hardened wax and soften it, allowing it to then drain out of the ear.

We did not find any evidence that water‐based or oil‐based drops were any different to saline or water. However, we also did not find any evidence that water or saline were better than doing nothing. Typically, earwax comes out on its own when you are sleeping. However, anything inserted into the ear could prevent the substance from exiting and even push the earwax deeper into the ear canal. A momentary loss of hearing may result from excessive ear wax. Additionally, it may cause vertigo, ringing in the ears, ear pain, itching, disorientation, and even an ear infection. The antibacterial and antifungal drug chlorobutanol thins down ear oils to make them easier to insert into the ear canal. This review includes 10 studies comparing the effects of ear drops for the removal of ear wax. The drops were of a variety of different types: oil‐based treatments, water‐based treatments and others, and the studies looked at both active comparators such as saline or water alone and no treatment. Prescriptions must be written on an Authority Prescription Form, and the approval number must be noted on the prescription. Pharmacists cannot dispense the item as a pharmaceutical benefit unless it has been approved by Medicare Australia (indicated by the presence of the approval number). We found no evidence of a difference in the proportion of patients (or ears) with complete clearance of ear wax when the active treatment group was compared to the water or saline group (RR 1.47, 95% CI 0.79 to 2.75; three studies; 213 participants; 257 ears) ( low‐quality evidence). Two studies applied drops for five days, but one study only applied the drops for 15 minutes. When we excluded this study in a sensitivity analysis it did not change the result. Cerumol works well to dissolve ear wax obstructions. If you must choose between a few drops, most studies have shown that none is superior to the others. One review concluded that none of these products was superior to sterile saline or saline water. Six studies (360 participants; 491 ears) contributed quantitative data and were included in our meta‐analyses. Adults, the elderly and children over 1 year: Tilt head sideways with the affected ear facing upwards. Avoid touching the ear with the dropper. Using the dropper, place 2-3 drops of the oil at room temperature into the ear, then gently massage around the outside of the ear. Never insert the dropper or any solid object into the ear. Allow a few minutes for the drops to reach the eardrum by keeping the head tilted and repeat in the other ear if required. Plug the ear lightly with cotton wool if necessary to prevent leakage. Use the drops twice a day for up to seven days. Ear drops are an affordable and effective treatment. A drop or two of specially-formulated liquid, left to work for a few minutes several times a week, is usually enough to make even hardened ear wax disintegrate and come out of your ear naturally – without the need to push foreign objects into your ear canal.
