McDonaldland takes place predominantly in its own high fantasy world, however parts of the McDonaldland world have low fantasy interactions with the real world. In earlier commercials, McDonaldland had sentient trees that grew apple pies, a hamburger patch, a French Fry patch, a shake volcano, Filet-O-Fish Lake, and the one-shot Sundae Mountain. Merchandise featuring the McDonaldland characters included a kid-friendly magazine titled "McDonaldland Fun Times", which published six issues a year. A direct-to-video animated film titled The Adventures of Ronald McDonald: McDonaldland Treasure Island was released in 1989, featuring much of the McDonaldland characters from the 1980s. Video games featuring the characters were also released, such as M.C. Kids and McDonald's Treasure Land Adventure. People were, of course, curious to know what a Grimace shake tastes like. What flavor is a purple, triangular, googly-eyed monster, who might actually be a taste bud? Turns out he’s “berry” flavored. One reporter compared the taste of the shake to cereal-milk. Mayor McCheese makes appearances in three episodes of the Fox animated television series Family Guy: " Believe It or Not, Joe's Walking on Air", " Road to the Multiverse" and " Valentine's Day in Quahog". In all three episodes, he is voiced by John Viener. Ronald McDonald appears during a child's birthday party at a McDonald's in one scene of the film Mac & Me. He is portrayed by Squire Fridell, who played Ronald in commercials during the latter half of the 1980s.

The most often occurring regions, as seen in numerous McDonaldland commercials and video games are a forest, town, sea, and outer space. Their exact geographic layout is unknown, but within these regions are several locales including Grimace Island, a Western-themed area, several portals to real world McDonald's restaurants, the Moon, and Ronald's home. Grimmis is an Aspect of Death entitling Sacrifice, and is the brother of Esquire, the Aspect of Death entitling Nothingness. Grimmis was in life, one of Death's lesser aspects that worked to claim souls who have transpired to Death's dimension. He wished to attain greater power and become a more powerful Aspect, and sought any opportunity he could to do so. In addition to being used in advertising, the characters were used as the basis for themed equipment in the "PlayPlaces" attached to some McDonald's outlets, small recreational activity rooms intended for young children. While the McDonaldland portion has received somewhat less attention since 2003, several of the characters including Ronald McDonald, Grimace, Birdie the Early Bird, and the Hamburglar are still seen in commercials, Happy Meal toys, and materials supporting the Ronald McDonald House Charities. The Fry Kids and McNugget Buddies received entire dedicated McDonaldland themed line-ups of Funko Pop! figures in 2019 and onward, other characters that appeared in the line-up include Ronald, Grimace, the Hamburglar, Birdie the Early Bird, Mayor McCheese and Officer Big Mac. He is the Aspect of Death who is responsible for attempting to take Deathlius Loss'end's soul, though failed to do so, and ended up losing his life because of it. This event is looked upon as a "sin" of Death.

The line between hero and villain is paper-thin — something Grimace (and McDonald’s) is learning first-hand. On June 12, McDonald’s released the berry-flavored Grimace shake as part of the extensive marketing campaign to celebrate the mascot’s birthday. The energetic response from fans turned the promotion into a celebration of Grimace as a queer icon, with people dressing in blursed cosplay for social-media posts about the birthday, and creating fan art of him and the Hamburglar attending Pride. However, only two weeks later, a search for Grimace on TikTok doesn’t surface people honoring “ mother Grimace.” Instead, it brings up videos where Grimace’s shake isn’t a purple treat, but rather a deadly concoction that kills or incapacitates all who dare drink it. Though many assume it had something to do with the lawsuit, this might be incorrect as Mayor McCheese and Officer Big Mac both appear in the 2020 Funko McDonaldland Pop! Vinyl! figures. One day, during the Battle of Glaecings, Grimmis looked upon the distraught and destroyed state of Deathlius Loss'end, and proclaimed that his time is come to give Death his soul. Whilst in the middle of doing so, Deathlius refused to yield to the Aspect, and killed him where he stood. Because of this, Deathlius lives, but his soul is forever torn by Grimmis' magic. Death didn't take this too kindly, seeing as Deathlius killed one of his "sons", he made Deathlius' weapon into a harvester to reclaim the souls lost by Grimmis. Later on though, Deathlius would rid himself of being Death's pawn.

When was the McDonald's character Grimace introduced?

Grimace originally debuted as a large purple being with four arms and two legs who loved milkshakes. After that first campaign, Grimace was reintroduced in his modern form, with his number of arms reduced by two. Grimmis was created by Death as an Aspect, to entitle his being as Sacrifice. He was created as an aspect roughly the same as Esquire, who represents Nothingness. Esquire killed Krivion who represented Sacrifice beforehand, for "getting in his way". The modern commercials are really cool and merchandise generally portray him as a well-meaning simpleton, whose clumsy antics provide a comic foil to Ronald McDonald. The character was retained after the streamlining of the characters in the 1980s and was one of the few members to also be retained until the end of the McDonaldland commercials. Grimace in the early days was used to advertise the happy meal. Grimmis wore the usual Aspect armor, but transformed it into a knight sort of theme. He was fantasized by the knights in the Three Wars of Outhria between the Brotherhood and Confederacy. He wields a one handed sword which he drops in his encounter.
